Aim: Recent estimates of HIV prevalence among women of child bearing age in South Africa exceed 30%. Vulnerable women living with HIV or at risk of HIV in SA face barriers to protecting themselves. Gender-based violence and alcohol and other drug (AOD) use reduce the likelihood of testing for HIV, access to antiretroviral therapy (ART) among those who test positive, and adherence among those who are on ART.Beginning in 2001, we adapted and tested a comprehensive evidence-based gender-specific intervention for AOD-using women ~the Women’s Health CoOp (WHC) in several districts of South Africa.

Method / Issue: The intervention, addresses these intersecting contextual and risk factors. It has been packaged, and it is ready for implementation and dissemination to reach more AOD-using women. We present the rationale and methods surrounding the history and future in implementation science and the necessary steps being taken to implement the WHC in antenatal and health clinics, and substance abuse rehab programmes.

Results / Comments: By conducting an implementation science study with the WHC intervention, we can test the scalability in real-world settings with providers and patients. Such an implementation evaluation will determine acceptability, adoption, cost, fidelity, penetration, and sustainability, in addition to service outcomes (comprehensive services and timely service linkages). Patient outcomes on the programme’s effectiveness and satisfaction in the setting are also measured.

Discussion: Our mixed methods approach is essential to determine if the WHC can be implemented with fidelity in a sustainable manner in these South African clinics while maintaining its effectiveness. We will also estimate its public health impact.
