Aim:
To explore the current rehabilitation framework of PLHIV at a semi-rural setting in South Africa and to initiate the development of an alternative model of care in rehabilitation to provide a seamless, coordinated delivery of rehabilitation service throughout the health spectrum.
 
Method / Issue:
A focus group discussion with various stakeholders including PLHIV, the multi-disciplinary healthcare team and community outreach partners at a semi-rural hospital in KwaZulu-Natal, South Africa using Van Manen?s pedagogy on interpretive phenomenology as the conceptual framework.
 
Results / Comments:
Barriers to rehabilitation included fiscal challenges, environmental construct and structural limitations. The development of this model offered to PLHIV seems to theoretically provide an alternative rehabilitative approach to alleviate the current burden on the health system, while at the same time offering a consistent, well-coordinated and seamless service.
 
Discussion:
HIV continues to challenge healthcare professionals even after the rollout of antiretroviral therapy. South Africa, among the worst affected countries in the world by the pandemic, has seen the effect of people living longer but facing disabling effects of both the virus and the associated impairments of the antiretroviral therapy. Rehabilitation within the evolving context of the disease has changed its focus from the impairment of the individual to the participation restrictions within a person?s daily life. The results of this study show barriers to rehabilitation at various levels of care, including funding structures and multi-stakeholder team interaction. To provide a continuum of coordinated, multi- level, multi-disciplinary and evidence-based service within rehabilitation, the development of a model of care within the healthcare structure is required. The proposed model of care in rehabilitation for people living with HIV in a healthcare setting is aimed to provide an example of a continuum of coordinated service throughout the disease trajectory. The assumption is that the burden on the healthcare system will be curbed and the projected benefit for all stakeholders will promote a sought-after rehabilitation model for PLHIV in resource-poor settings.
