Aim:
Since December 2009 Public Health Authorities of the Madrid Autonomous region have been promoting the development of a Service of Prevention and Early Diagnosis of HIV and syphilis, set in eight primary care centers, prioritized with respect to HIV epidemiological criteria. These services provide counseling and rapid-HIV tests and they are particularly aimed at vulnerable populations. The objectives of this study were to analyze the results of a program offering rapid-HIV testing in socio-culturally adapted services located in selected primary care centers and to identify factors associated with a reactive result.
 
Method / Issue:
Descriptive cross-sectional study. We analyzed data from consultations that took place between 29th April 2010 and 31st May 2012. We described the population and stratified it into three groups: men who have sex with men (MSM), heterosexual men (HM) and women. We compared the distribution of the variables (age, origin, education, sexual history and risk behaviors), assessing differences between groups using a two-sided independent t-test and chi-square statistic. Factors associated with first time HIV testing were analyzed by logistic regression.
 
Results / Comments:
Over a 2-year period, 1940 consults were registered. Of these, 493 (25.4%) were MSM, 874 (45.1%) were HM and 573 (29.5%) were women. The mean age was higher in HM (34.4 ± 10.8, p < 0.001). By country of birth, 35.4% were immigrant population. About 55% had a steady partner. The use of condom was less frequent in women: 24.1% did not use it regularly, compared to 14.9% HM and 11.4% MSM. Of the total, 1826 required the realization of rapid-HIV test (94.1%). It was reactive in 41 occasions and indeterminate in other 5, which is a reactive rate of 2.24%. A rapid-HIV test reactive was more common among MSM (6.3% compared to 0.7% in HM and women), in the group of 35-44 years (4.0%), among immigrants (2.5%), and in users with less level of studies. In the multivariate analysis the risk of reactive result in the rapid-HIV test was associated to MSM (ORa: 6.97, CI95%:2.21-21.95), not using condom regularly (ORa: 3.94, CI95%:1.63-9.48), and seropositive sexual partner (ORa: 3.05, CI95%:1.09-8.48).
 
Discussion:
Services were used by a large percentage of MSM and immigrants, two groups considered vulnerable to HIV. Uptake activity by cultural mediators is reflected especially in attracting these populations. The detection rate of HIV was high. The physical location of services and its target population conditioned clear differences in the profile of users and higher reactive test rates compared to other programs.
