Aim:
Show the importance of nutritional care in the care offered to PLWHA in Côte d?Ivoire.
 
Method / Issue:
This is a longitudinal study on a cohort of 8700 PLWHA adults followed during 24 months (October 1st, 2009 to October 1st, 2011) in 27 health facilities spread on the entire area of the country. The 8700 PLWHA received nutritional counseling followed by a systematic evaluation of their nutritional status in the different health facilities concerned with the standardized anthropometric tools and standard procedures for malnutrition diagnosis. The body mass index (BMI) was the only parameter considered in the diagnosis of malnutrition in this study and therapeutic foods used were the CSB (corn-soya-blend t) and PLUMPY NUT.
 
Results / Comments:
2277 PLWHA have been diagnosed malnourished with a BMI between 16.5 and 18.5 on average, 26% of the cohort total. 921 on the 2277 have been diagnosed severely malnourished with BMI < 16.5 on average, more than 40% of the total of malnourished. Only 186 PLWHA on the 921, or 20% of severely malnourished received therapeutic food during 60 days. Weight recovery have been observed in 180 of the 186 PLWHA with a BMI ranging from 19 to19.5 57 deaths. Otherwise 678 PLWHA on the 921 diagnosed severely malnourished missed at least one monthly appointment for the renewal of their ART, influencing thus the quality of treatment adherence.
 
Discussion:
Recognizing that adequate nutrition is regarded as the most critical need of PLWHA, it is imperative that the underdeveloped countries integrate it into their Care package, since the majority of PLWHA diagnosed malnourished according to this 2007 study in Abidjan lacked financial resources to feed themselves properly, which had the effect of altering the adherence to ARV treatment for these patients. This is an opportunity to point out that we have been confronted during this study period to two major difficulties: the recurrent unavailability of therapeutic food at the national level which hinders the good running of activities with consequence: PLWHA diagnosed malnourished but without treatment. The second problem has been the absence of reliable statistical data which could make the link between non-adherence to treatment observed among 678 PLWHA and severe malnutrition diagnosed among these persons.
