Aim:
Recent literature in implementation science has revealed a gap between research and efficient innovative change in clinical practice. To learn how to effectively transfer research findings into innovation of health service, we need a fully detailed understanding of the process of innovation dissemination from research to clinical practice. We launched a theory-based HIV disclosure program for parents living with HIV/AIDS (PLH) in China, with an aim to integrate consulting service on parental disclosure (i.e., PLH make a planned, developmentally appropriate disclosure to their HIV seronegative children) into routine clinical practice in HIV clinics. The specific objectives for our research team at the preparation stage focused on information exchange with local health care providers and PLH and capacity building of local partners. We will delineate lessons learned at the preparation stage for this project from the perspective of Diffusion of Innovations Theory.
 
Method / Issue:
The key activities to promote the innovation dissemination included: 1, developed a partnership with multiple stakeholders to support the project. 2, developed readable intervention curriculum and training manual for intervention facilitators, as well as booklets for PLH. 3, provided project facilitators an interactive participatory-approach training workshop, using multimedia learning materials. 4, set up an internet-based information exchange platform for the project network composed of research coordinators and facilitators.
 
Results / Comments:
According to diffusion theory, an innovation dissemination process is shaped by three attributes of the innovation: the relative advantage (perceived benefits), compatibility (the degree to which an innovation match the needs and capacity of potential adopters), and complexity (the degree to which an innovation is as difficult to understand and use). Corresponding to these attributes, we need to 1) demonstrate the significance of project and benefits for the target population to improve buy-ins of multiple stakeholders; 2) tailor intervention activities so that they are well matched to the needs and capabilities of heath care workers and clinical environment and acceptable to target population; 3) transform original text curriculum into multimedia learning modules in training workshop to illustrate complicated issues, and develop user-friendly curriculum packages for various audiences. In addition, the innovation dissemination is a process of continued information exchange and reinvention (adaption). There is no one-size-fits-all innovation template. Therefore we need to adopt a participatory training approach to encourage trainees? sharing their experience and opinions and build an online interactive platform to discuss specific clinical problems, to exchange updated information and provide on-going technique support.
