Aim: In côte d?Ivoire with a HIV prevalence of 4.6%, a PMTCT program has been implemented in 2005. There is a very low utilization of maternal services (<10%). In rural area, instead of going to clinic, pregnant women prefer to see CTBA at home when needed.
Increase PMTCT services attendance for 05 maternal clinics in Guiglo, Blolequin and Toulepleu districts, in West health region of côte d?ivoire.

Method / Issue: Pilot project designed to be implemented in 09 maternal clinics in 3 districts for 24 months, from April 2012 to march 2014. Strategies based on organization of training session on HIV prevention for CTBA, CBO members and health workers. CTBA had to visit pregnant women regularly at home in order to offer them HIV pre-counseling and accompany them to maternal clinics for comprehensive PMTCT services. Implementation of activities and data monitoring have been ensured by CBO members.

Results / Comments: Preliminary results in the 3rd quarter: we have 27 CTBA and 03 CBO members have been trained. We note 2951 antenatal and 273 postnatal visits in clinic accompanied by CTM after HIV pre-counseling, increasing the attendance rate (51%). 1236 women received Counseling and Testing and 19 tested HIV-positive, benefited from PMTCT services. Among 729 women who delivery in maternal clinics, 58% were accompanied by a CTBA. We note a decreasing percentage of women who delivery at home (from 41 % to 13%).

Discussion: CTBA play an important role in following the pregnant women at rural area. Through this pilot, building their capacities and involving them in PMTCT activities, is an opportunity to enhance qualities of maternal and infant health. Moreover, scaling up of this project, remain a challenge for local health authorities.
