Aim: To raise awarennes about the challenge in Kenya is the cost of genotypic testing, in particular, the reagents, which puts access totally beyond the reach of the ordinary citizen.

Method / Issue: Nyumbani Diagnostic Laboratory (NDL), awaiting ISO 15189 accreditation, has been, over the past 15 years, core in the management of over 10,000 children perinatally infected with HIV. The children are cared for either in a residential facility, or through a community-based program. Founded in 1998, NDL has developed from providing CD4 and viral load testing in the beginning to offering genotypic testing for HIV drug resistance since 2010 and is the first and only Diagnostic Laboratory offering this service in Kenya. Access to resistance testing for the children began in 2005 through a collaboration with Kanazawa University, Japan. Genotypic testing for HIV drug resistance has now made it possible to move from blind prescription of antiretroviral (ARV) medication to informed prescription with resultant improved health for the children and regimen longevity. With the possibility of an already resistant virus being transmitted, informed prescription of ARV regimens is crucial. The challenge in Kenya is the cost of genotypic testing, in particular, the reagents, which puts access totally beyond the reach of the ordinary citizen.
